We have received almost everyone’s paperwork, and we thank you! However, there are still some families that haven’t yet turned theirs in.
The State of Colorado requires that all files contain the following paperwork:
- A completed and signed contract (to be updated yearly)
- Up-to-date shot records (due on the first day of care)
- A health appraisal filled out and signed by your child’s physician (due within the first 30 days of enrollment and then yearly thereafter)
- Income eligibility form (USDA Food Program Requirement to be updated yearly)
- Topical Procedures form
- Enrollment form with emergency information
- Media Use form
- Child Abuse Reporting form
- Illness Policy form
